At St. Bartholomew's Court Nursing Home we aim to work in partnership with Knowsley Metropolitan Borough Council/Social Services and the hospital to offer intermediate care for a 2- 4 week period.

Our aim is to offer a homely environment and provide 24 hour nursing care to compliment the rehabilitation process and support discharge planning.

The Intermediate Care Multidisciplinary team will establish the patient's suitability following the agreed criteria for the programme prior to admission to St. Bartholomew's Court.

The Occupational Therapist or Physiotherapist will then send the information electronically via Single Point of Access NHS email to St. Bartholomew's Court Nursing Staff NHS email regarding the client's initial assessment. The Primary Care 24 Doctor and the Nursing staff including the Matron review the documentation. A multi-disciplinary approach is then used to come to a decision whether the patient can be safely allocated an intermediate care placement.

The patient's own G.P. or a hospital Medical Officer will ensure the patient's medical suitability and will email the nursing staff at St. Bartholomew's Court to verify this, and share with the medical team.

St. Bartholomew's Court admission procedure will then be followed.

For the 2-4 week period of rehabilitation, medical care will be provided by the Primary Care 24 GP, who will visit daily Monday - Friday, 09:00am - 6:00pm. Out of these hours the GP is accessed via the 'out of hours' service.

Multi-disciplinary Team meetings between the Occupational Therapist, Physiotherapist, Social Worker, Community Pharmacist, G.P. and Nursing Staff will be held at St. Bartholomew's Court on a weekly basis to chart the resident's progress and plan for discharge. There is also a daily huddle that takes place to ensure all involved are aware of daily information and plans are shared.

The Therapy Staff are the Key Workers and part of their role is to liaise with family and services when planning progress and discharges. The GP and nursing staff manage the care and medical needs and ensure medications and discharge letters are completed and any medical equipment required or district nursing referrals are in place upon discharge.
